Registration for all new students occurs at the Central Registrar’s Office, located at 962 Luther Road East Greenbush, NY 12061-3900. (518-207-2062). Registration hours are from 8:00 a.m. to 4:00 p.m., Monday through Friday, BY APPOINTMENT ONLY. Please call to request a registration packet. When this packet is complete, please call again to set up an appointment.
Student Change of Address
Address changes, even for students who move to a new location within their building attendance zone, must be handled through Central Registration in the Office of Pupil Personnel Services. Please call the office at 518-207-2062 for information and to set up an appointment. Do not call the transportation department. All change of address information must be coordinated through Central Registration.
New Student Registration – Required Documents
One of the following:
A. Deed or Mortgage
If you are not the owner of the house, but live with the owner such as a grandparent of the child, please call 207-2062 for further instructions. You will need to provide additional documentation.
If you rent an apartment or house, your child’s name must appear on the lease or lease application. If the child’s name is not listed, then in addition to the lease, you must also bring a letter from the landlord stating that the child lives at that address. The letter must include the landlord’s name and phone number.
C. Contract to Build/Buy a Home
In addition to the above, two (2) more proofs of residency are required. Any two of the following are acceptable:
a. Bank account and/or credit union statement
c. Automobile and/or homeowners/renters insurance policy
d. Automobile registration (NOTE: A driver’s license is not acceptable)
e. Telephone, cable and/or utility bills
• Child’s birth certificate (original with seal)
• Most recent custody papers
• Adoption papers
• Court ordered assignment
• Social Services assignment or agency assignment
• Proof of district residency (A, B, or C and 2 of items a-e)
• Proof of child’s immunizations
Documents must be current and original – they will be copied and returned to you immediately. Printouts from online accounts will be accepted if they show the name and address of the resident and have a current date.
Supplemental documentation required for school registration:
• IEP (Individualized Education Plan) from previous school district
• Section 504 documentation from previous school district
• Last report card for placement purposes, transcript from previous school
• Foster care student – DSS2999 form
A health physical must be provided in order to attend school. The exam must have been performed within the 12 months prior to the child’s start date in our school district. If a physical has not been performed, you are allowed 30 days from the start of school to obtain one. The Health Certificate/Appraisal form is provided for your doctor’s use. Please remember, however, that the most current immunization records are absolutely required in order to register.
Required Student Immunizations:
Under New York State’s immunization law, students may not attend school unless they provide acceptable written proof (including dates) of immunizations that has been prepared by a physician. Section 2164 of the New York State Public Health Law requires that all children receive the following:
• 3 to 5 doses of diptheria, tetanus, and pertussis
• 3 to 5 doses of oral polio
• 2 doses of measles, mumps, and rubella (students under age 7 -- require one dose)
• 3 doses of hepatitis B
Students entering Kindergarten or 6th grade in the 2014-2015 school year will now be required to have two doses of the varicella (chicken pox) vaccine and 3-4 doses of poliomyelitis vaccine. All other students will be able to attend school this year with one dose of varicella, and three doses of poliomyelitis. This new requirement will apply to all 2014-2015 incoming Kindergarten and 6th grade students, along with the subsequent grade levels that the 2014-2015 Kindergarten and 6th grade students move up to.
Students in grades pre-K or K through 6 will be required to have age-appropriate doses of DPT or DTaP, in accordance with the ACIP schedule.
Two doses of MMR are no longer required for entrance in to Kindergarten, though students must have two doses by age 7.
A student who is in process is defined as one who has had the first dose of all required immunization series and has appointments to complete the series in accordance with the ACIP catch up schedule. See http://www.cdc.gov/vaccines/schedules/hcp/child-adolescent.html. A student who is in process must be allowed to attend school.
Students may present proof of immunity by serology (blood test) to the following immunizations: measles, mumps, rubella, hepatitis B, varicella, and poliomyelitis.
Medical exemptions to immunizations must be reissued annually. The written exemption must identify the immunization exempted, the medical contraindication for the exemption, and the length of the time immunization is contraindicated. Once you have compiled all of the required documents in STEPS 1-4 please call Central Registration at 207-2062 to schedule a registration appointment.
High School Students:
Once registered, new secondary school students must make an appointment with their guidance counselor to schedule their classes. It is suggested that secondary students bring their report cards or a copy of their transcript with them in order to facilitate the scheduling of appropriate classes.
To be eligible for kindergarten at the opening of school of any year, a child must be five years of age on or before December 1. If your child is between the ages of birth and five, please call 207-2064.
Registration takes place beginning in December for the following school year.
For September 2015 Kindergarten enrollment, your child must turn five (5) by December 1, 2015.
Kindergarten registration for 2015 (future class of 2028) is upon us. Please call the school your child will attend to have the registration packet mailed to you. If you are not sure which school they will attend, call the Transportation Department. For additional information and registration dates click here.
Kindergarten registration takes place in the month of February at each of the elementary buildings. To register, children must be five (5) years old on or before December 1st of the year they will start school. Kindergarten registration packets will be mailed out in January to all eligible families currently in the district database.
If you have not filled out a “School Census” form please do so. This will enter your child into the districts database, but does not register them. We encourage you to use the school census for all children not enrolled in school who live in your home.
Registration for kindergarten is ongoing throughout the year once we have finished the February dates held at each building. Parents are encouraged to register kindergarten students by March 1st. Once your child is registered for kindergarten, a screening will be scheduled for your child at their home elementary school by appointment in the spring. You will receive notice of that in the mail once you have completed the registration process.
Private School Students:
When a student is enrolled into a private school, who is an East Greenbush Central School District resident, it is necessary that they register through the school district Registrar’s Office. Private school students must be registered through the district in order to receive transportation services. The district also provides payment for health services and books to private school students. Registration ensures us the information necessary to be fiscally accurate in providing those services.
