All students who wish to play interscholastic sports this fall must be certified by the school nurse at a Health History Review in August. The school nurse will review each student’s health record and notify them whether a physical exam (to be done at a later date) is necessary.
Each fall sport has an assigned date and timeframe for Health History Reviews in the Columbia Health Office, beginning with football and cheerleading on Wednesday, August 1. (There will be make-up dates for those who cannot attend their sport’s assigned day.)

Does my child need to have a Health History Review before playing a sport this fall?
Yes. All students must be certified by the Health Office before starting competition.
What is the difference between a Health History Review and a physical exam?
A Health History Review is mandatory for all students before they compete in school sports. It includes a review of the student’s health history and interview by the school nurse. If necessary, the student will be informed of any additional information required before they can begin practice. This may include a physical exam, which can be scheduled for a later date with the school or a student’s primary physician.
How can I schedule a physical exam with the school?
It is recommended that your family physician conduct your child’s physical exam because they are more familiar with their health history. However, the school does provide physical exams prior to interscholastic sport competition. If you would like to schedule a physical exam with the school, contact the appropriate school health office (Columbia 518-207-2070 and Goff 518-477-2731).
If my child has already been certified, does he/she need to attend the Health History Review in August?
Yes. Participation in more than one sport requires re-certification. For example, if your child played a sport in Spring 2018, they must be re-certified for any subsequent sport.
How long will the Health History Review take?
Approximately 10-20 minutes.
