Immunizations

All newly entering students are required to have a physical evaluation. This may be done by your physician or at school. We may accept a physical that was done within the last 12 months of the student entering. If you have a school physical, you will be notified of any problems noted during the school health assessment. If your child has a vision concern and is seen by a doctor, please send in an evaluation, the results will be kept in your child’s cumulative health record. Under section 2164 of the NYS Public Health Law, all children attending school must be immunized and present acceptable proof. *

Minimum vaccine requirements include:

  • Three to five (3-5) doses of diphtheria, tetanus and pertussis
  • One (1) dose of tetanus and diphtheria toxoid-containing vaccine and pertussis vaccine booster Tdap (required for 6-12 grades)
  • Three to four (3-4) doses of IPV/OPV
  • Two (2) doses of measles, mumps and rubella
  • Three (3) doses of Hepatitis B
  • One to two (1-2) doses of Varicella (chickenpox) vaccine
  • One dose of menactra (meningitis) for grade 7 and grade 12 (grade 12 dose must be given after the age of 16)

*Acceptable proof consists of a list of all dates signed or stamped by a physician OR a certificate from a clinic. If you have any specific concerns about your child’s health, please visit or call your child’s Health Office.
