Under section 2164 of the NYS Public Health Law, all children attending school must be immunized and present acceptable proof. *
Minimum vaccine requirements include:
- Three to five (3-5) doses of diphtheria, tetanus and pertussis
- One (1) dose of tetanus and diphtheria toxoid-containing vaccine and pertussis vaccine booster Tdap (required for 6-12 grades)
- Three to four (3-4) doses of IPV/OPV
- Two (2) doses of measles, mumps and rubella
- Three (3) doses of Hepatitis B
- One to two (1-2) doses of Varicella (chickenpox) vaccine
- One dose of menactra (meningitis) for grade 7 and grade 12 (grade 12 dose must be given after the age of 16)
