Immunizations

Under section 2164 of the NYS Public Health Law, all children attending school must be immunized and present acceptable proof. *

Minimum vaccine requirements include:

  • Three to five (3-5) doses of diphtheria, tetanus and pertussis
  • One (1) dose of tetanus and diphtheria toxoid-containing vaccine and pertussis vaccine booster Tdap (required for 6-12 grades)
  • Three to four (3-4) doses of IPV/OPV
  • Two (2) doses of measles, mumps and rubella
  • Three (3) doses of Hepatitis B
  • One to two (1-2) doses of Varicella (chickenpox) vaccine
  • One dose of menactra (meningitis) for grade 7 and grade 12 (grade 12 dose must be given after the age of 16)

*Acceptable proof consists of a list of all dates signed or stamped by a physician OR a certificate from a clinic. If you have any specific concerns about your child’s health, please visit or call your child’s Health Office.
