NYS Screening and Health Exam Requirements for Middle School
New Entrants to the EGCSD: Health exam must be submitted within 30 days with current vaccination record. Screening of hearing, vision (near, far, color) and scoliosis will be done by the school nurse if not documented on the health exam. A dental exam is requested.
Grade 6: No mandated screening.
Grade 7: Health exam must be submitted. Dental exam is requested. Screening of hearing, vision (near and far) and scoliosis will be done by the school nurse if not documented on the health exam.
Grade 8: No mandated screening.
